1/31/2024 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, JaCaeri showed a lack of consideration for QMHP-C and the session, due to personal conflicts and external distractions. JaCaeri reframed from communicating his needs and expressing his feelings at which mother reports these behaviors last up to 5-10 minutes in duration. Behavior: JaCaeri refuses to participate or contribute to family discussion. JaCaeri constantly complains and make excuses for his behavior playing victim and blaming others for his mistakes. JaCaeri was seen exhibiting low self-esteem, as he was involved in his own feelings, thoughts which behaviors last up to 2-5 minutes in duration. Intervention: QMHP-C focused on JaCaeriâs ISP objectives and what he needs to do in order to achieve them. QMHP-C offered new ways to heighten JaCaeri awareness of his ill-mannered behaviors, as well as helping him with functioning more constructively within his daily life. QMHP-C encouraged JaCaeri to present healthier habits and routine by challenging himself to make better choices and try new things. QMHP-C examined and evaluated JaCaeri responses to feedback. QMHP-C questioned JaCaeri on the statements he made about having difficulty expressing his feelings. Intervention: QMHP-C reviewed strategies targeted at helping JaCaeri to work through his difficulties expressing his anger. QMHP-C utilized age appropriate mood regulation skills and techniques, as a way to help JaCaeriâs destructive and critical demeanor, as well as to assist him with regaining and regulating his uncontrollable conduct. QMHP-C encouraged JaCaeri to be mindful of others personal space. QMHP-C encouraged JaCaeri to refrain from displaying negative behaviors towards his mom, to talk calmly and express himself without aggression. QMHP-C suggested JaCaeri wash his hands, wear a mask, and be mindful of others. Response: JaCaeri was reluctant and hesitant. JaCaeri did not offer any forms of social or friendly rapport back to the QMHP-C, remaining unemotional displaying an unresponsive and nonchalant attitude throughout the session. JaCaeri took over 15 minutes to reduce hyperactive behaviors. JaCaeri became more impulsive and started to cause more disruptions in the home. JaCaeri expressed his difficulties in certain situations, displaying a negative tone when explaining why he dislikes sharing spaces with his younger siblings because they make everything more complicated than it has to be. Response: JaCaeri required assistance as he was unable to lower his voice and talk to authority with kindness and respect. JaCaeri showed lack of concern and empathy in response to positive cognitions. JaCaeri did not express verbal communication when he was prompted to apologize. JaCaeri had difficulty relaxing and following QMHP-C prompts. JaCaeri continued aggressive behaviors. JaCaeri did not extend a sociable rapport back to the QMHP-C. JaCaeri required redirecting from QMHP-C to reduce his voice volume 2-3 times.
